Master's Degree Requirements:
- PSA Birth Certificate
- Undergrad Transcript of Records from former school
- Marriage Certificate(if married)
Doctorate Requirements:
- PSA Birth Certificate
- MA Transcript of Records
- Marriage Certificate(if married)
Enrollment Guide

Visit the Dean's Office for requirement submission and evaluation

Secure Enrollment Form and proceed to the Registrar's Office for subjects to be enrolled and let it be signed by the Dean

Proceed to the Treasurer's Office for assessment
4
Give a copy of the Enrollment form to the Registrar's Office and to the Dean's Office
