Junior High School Requirements:

  • Original PSA Birth Certificate
  • Certificate of Good Moral
  • Form-138 (Report Card from Grade 6)
  • 2x2 ID Picture

Senior High School Requirements:

  • Original PSA Birth Certificate
  • Good Moral
  • Form-138 (Report Card from Grade 10)
  • 2x2 ID Picture

For Transferees

  • Original PSA Birth Certificate
  • Report Card
  • Form 137

Enrollment Guide

Secure Enrollment Form from the Registrar's Office

Fill-out Enrollment Form and let it be signed by the Principal
3
Proceed to the Treasurer's Office for assessment
4
Give a copy of the enrollment form to the Registrar and 2 (two) copies to the Principal
